The week after College Application Month, site coordinators receive a link to a reporting survey to report their results.

To help you gather the necessary information and prepare your answers, this is the information you will be asked to provide:

    1. Your contact information.
    2. How many graduating seniors are in your school?
    3. How many graduating seniors participated in your College Application Month event?
    4. How many total applications were submitted during your College Application Month event? (Count each college applied to: if one SUNY application was submitted for 4 colleges, count as 4.)
    5.How many additional applications were begun but not yet submitted during your College Application Month event?
    6. Did more students submit applications because of College Application Month?
    7. What is the total number of fee waivers submitted by students at your College Application Month event?
    8. How many volunteers participated in your College Application Month event?
    9. Who participated as volunteers? Choose all that apply.
    10. Did you have enough volunteers?
    11. How helpful did you find the College Application Month resources?
    12. Was the timing of College Application Month appropriate?
    13. What activities did your school implement before and during College Application Month, and which were most successful?
